Case Management
& Brokerage Program
The goal of the Case Management & Brokerage Program is
to implement an individualised service, which facilitates
client’s independence and participation in the community for people with a disability.
This program delivers flexible, comprehensive
and outcome focussed, short to medium term services (6-12 months). Referrals are received and reviewed regularly by priority of need where vacancies exist. Once referral is accepted, a case manager is appointed. An assessment is done and an individualised case plan is developed. Case Managers will work with the client and their family to co-ordinate a case plane and link in with existing services, as well as purchase services through brokerage. The individualised case plan is reviewed regularly to address changing needs.
To be eligible for the
Program clients must reside in Fairfield, Bankstown or Liverpool
LGA’s and need to be in the 12-64 age bracket.

This service is suitable for:
Person with an intellectual disability aged between 12-64, who has no other services involved, who is at risk of entering the juvenile justice system, or becoming homeless, or ageing carers. Clients must have a primary diagnosis of Intellectual Disability.
