Case Management & Brokerage Program

The goal of the Case Management & Brokerage Program is to implement an individualised service, which facilitates client’s independence and participation in the community.

This program delivers flexible, comprehensive and outcome focussed, short to medium term services (6-12 months) for people with an intellectual disability.

The Program also assists clients with Dual Diagnosis and Acquired Brain Injury. To be eligible for the Program clients must reside in Fairfield, Bankstown or Liverpool LGA’s and need to be in the 12-64 age bracket.
