Woodville Alliance provides support coordination to people with an approved NDIS plan. This independent service is called Innovate Plus.
Information about Innovate Plus is available in three languages:
If you have an NDIS plan which includes funding for support coordination, you can choose Innovate Plus to provide your support coordination.
Support coordination is support to implement your NDIS plan. It is defined by the National Disability Insurance Agency (NDIA) as:
‘Assistance to strengthen participants abilities to coordinate and implement supports and participate more fully in the community. It can include initial assistance with linking participants with the right providers to meet their needs, assistance to source providers, coordinating a range of supports both funded and mainstream and building on informal supports, resolving points of crisis, parenting training and developing participant resilience in their own network and community.’
Support coordination is funded under the NDIS support category ‘Improved Life Choices’.
Our qualified, experienced and specialised team of support coordinators will support you to live the life you want.
For more information, go to the Innovate Plus website.
We are working with you, our client families, to assist you to prepare for the NDIS. Check our blog for more details.
What is the NDIS?
The National Disability Insurance Scheme (NDIS) is a new way of providing support for Australians with a permanent and significant disability, their families and carers. If you have a disability that affects how you take part in daily activities and you acquired your disability before the age of 65, the NDIS is for you.
The NDIS is designed to give choice and control to you, the person with a disability. It is a new way of doing things. If you are eligible for the NDIS you will have access to NDIS funding for reasonable and necessary supports that help you determine your own future and match your needs and goals — to access your community and increase your independence.
What is the NDIA?
The National Disability Agency (NDIA) is the agency that was set up to deliver the NDIS.
