How do I get a case worker to help for the Strong Minds program?
A referral form will need to be sent to us to review to make sure we are the right service for you. This can be filled out by the person needing support, parent, carer, school counsellor or a government or non-government support person. Or, if you want the support for your child or young person, birth to 18 years, you can phone us to discuss your referral on 9646 5516 between 9 am – 5 pm weekdays.
Once we have processed the referral, we will be in touch to conduct an assessment so that we can work out together what the plan should be for your child or young person.
